All non-immigrant visa applicants who wish to apply for a visa at a U.S. Consulate in Canada must complete the Form DS-156 Non-immigrant Visa Application on-line. The on-line DS-156 creates a bar code, but your information is not downloaded. You must still print out the completed Form DS-156 including the bar code and bring it along with all other required documents to your appointment.

- The Consulate is open weekdays from 9:00AM to 4:30PM, except American and Canadian holidays.
- Non-emergency services (passports, reports of birth, notarials, Social Security, ITIN, etc.) are handled by appointment only (tel: 418-692-2095). American citizens are welcome to enter the Consulate during all business hours, but workload and staffing levels limit the time and personnel we can spontaneously allocate to non-emergency matters.
Because the Consulate in Quebec City has limited staff engaged in a broad range of activities, visa applications are adjudicated only two days per week, usually (though not always) Mondays and Thursdays. Visa applications are by appointment only; and are scheduled between 9AM and 2PM. Approved visas are generally issued the afternoon of the followng business day. Certain applications may require more lengthy processing.
Information needed to address nearly all consular matters is available here at Amcits.com; on the Consulate website - quebec.usconsulate.gov; or on the Consulate's recorded information (tel: 418-692-2095). Staff is available to handle walk-in and telephone inquiries on Wednesdays, from 2PM to 4PM.
The deposit slip for your visa application fee consists of 3 pages. A separate set of deposit slips is required for each applicant. Take all three pages of the deposit slip(s) to any branch of Scotia Bank and pay the non-refundable U.S. visa application fee of US$131.00. The payment must be made in either US$ cash, a US$ certified check or a US$ money order; incoming mail deposits and wired deposits are not accepted. The bank teller will stamp all three copies of your receipt and should return two copies to you. You must submit two validated deposit receipts with your visa application.
The special pre-coded deposit slip can be downloaded from the link provided below. The Consulate cannot accept applications that do not contain two copies of your deposit receipt.
